More bug fixes for you all!

- The bubonis entity will now properly notify you if they have
successfully afflicted someone.
- You can no longer PULL THORNS FROM BODY while off equilibrium.
- Fixed portals being usable before they fully opened.
- You can no longer fish from the trees or skies, you must be on the
ground.
- Cleave can no longer be started with paresis.
- Fixed Cleave not working through shield and prismatic barrier.
- Glancing BELOW or DOWN from the skies will now show multiple items
of the same kind in the room instead of just one.
- You must now have balance and equilibrium in order to DRAW a heart
from a corpse.
- Fixed some gender issues with the FISHING CUT message.
- Laceration effects are now cleared upon death.
- Fixed the wrong name showing when accepting an arena challenge.
- Savagery Mastery now grants proficiency in the weapon you master.
